Our members enroll online through a special link only we can provide. We maintain a credit card on file for each member/family that is be billed on the 1st day of every month. Members can dis-enroll at any time, but if we’re notified less than 30 days before the next regular payment, the member payment, as well as any balance on your account will still be due. In addition, there may be a fee to re-enroll if former members decide to rejoin our practice, except in certain circumstances with approval from the provider.
